Individual stress risk assessment form template

Employers have a legal duty to protect employees from stress at work.

This risk assessment can be used to support discussions with individuals or teams to systematically work through the pressures and consider if these are a source of stress and, if so, if any action can be taken to reduce the risks to performance and health.

Individual risk assessments are facilitated by the Line Manager. It is useful if the individual employee has an opportunity to look through the risk assessment in advance of the one-on-one meeting.

Lone working risk assessment form template

Individuals are alone at work when they are on their own, they cannot be seen or heard by another worker, cannot expect a visit from another worker for some time and/or where assistance is not readily available when needed. Therefore, lone workers are those who work by themselves without close or direct supervision. 

Lone working is not against the law, however, the law does require you to carry out a risk assessment.

A separate lone working risk assessment is especially important because lone workers are often more vulnerable than someone who, for example, works in a busy office.

Medical declaration form template

You may wish for new employees (including those changing roles) to complete a medical declaration form prior to commencing employment. This process enables new starters to declare any health condition or disability which may affect their ability to do the job they have been offered. The form may be used in order to accommodate any disability or impairment which a candidate has declared.

Adjustments may include physical adaptations, or adjustments to working arrangements such as hours. Where the candidate has answers ‘Yes’ to any of the questions in the declaration, you might want to also involve an Occupational Health Service (OHS) who may provide further advice.

Young person's risk assessment form template

Under health and safety law, you mush assess the risks to young people under 18 years old, before they start work / work experience and tell them what the risks are. An employer must assess the risk of a person's ability and should take into account:

  • Inexperience
  • Lack of awareness of risk
  • Immaturity
  • Layout of working area
  • Form, range and use of equipment
  • Manual handling
  • Exposure to biological or chemical agents
  • Exposure to heat, noise and vibration
  • Extent of training required
