I can confirm that your request for parental leave has been granted. Your last day of employment prior to your parental leave is [date], and you must report back to work on [date].

The following benefits are suspended during parental leave, in addition to your regular pay: [list the suspended benefits]. In other respects, your contract of employment remains in place and your continuity of employment is not broken by your period of parental leave.
