Before completing the appropriate section of the Assessment Form, please refer to these notes for guidance.
A scoring of between 1 and 5 should be applied to each criteria. Please be objective in your assessment and ensure that each individual is awarded a score which accurately reflects their performance in each area. All assessment scores should be backed up with relevant comments. Use of extreme scores such as 1&amp;amp;amp;rsquo;s or 5&amp;amp;amp;rsquo;s must be justified using relevant comments and examples.
