Before completing the appropriate section of the Assessment Form, please refer to these notes for guidance.

A scoring of between 1 and 5 should be applied to each criteria. Please be objective in your assessment and ensure that each individual is awarded a score which accurately reflects their performance in each area. All assessment scores should be backed up with relevant comments. Use of extreme scores such as 1’s or 5’s must be justified using relevant comments and examples.
