I refer to the disciplinary hearing on [date] concerning your unsatisfactory number of absences from work.

Following a review of your attendance record, discussions with you and a number of warnings, you were given a final written warning on [date] following the above hearing. In that letter you were warned that if your attendance record did not improve within [time period] [to a satisfactory standard], [Company] would terminate your employment.

At the disciplinary hearing on [date], we discussed your continued absences and you could give us no assurance that your attendance would improve in the future.

I regret that [Company] has therefore decided to terminate your employment on [date] at the end of your [number of weeks] notice period.
