Disciplinary hearing

Thanks for attending.

[Purpose of the meeting]

To address the allegations set out in my letter inviting you to this hearing.

To give you an opportunity to respond, and put forward any mitigating circumstances you wish to be taken into account. [If you have chosen to be accompanied at the hearing by a colleague or trade union official they may ask questions or read a statement on your behalf, but cannot answer questions for you].

Once you have responded and made any statement you wish to make, we will then adjourn the meeting and I will decide what level of disciplinary sanction to impose (if any). This will be notified to you in writing.

The hearing is being convened to consider [Provide details of allegations / evidence as set out in the invite letter].

Now is your opportunity to respond and to put forward any mitigating circumstances you wish to be taken into account before I make a decision.

Is there anything further that you would like to add?

[End meeting]

Thank you again for attending, I will take into account everything you have said, and will consider my decision.
